Welcome. This online form will guide you through the process of ordering a Pre-Paid Debit Card for your Capital Security Bank 'CSB' account, please complete all required sections. This form can be used to order multiple cards to be sent to the same delivery address however a separate form will need to be completed for each card being delivered to a separate address. Only CSB account holders can apply for a pre-paid debit card. If you are not a CSB account holder and would like to be please click here to get started. Please enter your current residential address details. Please note that if your residential address has changed since you opened your CSB account or if the proof of address CSB holds on file is older than 3 months from the issue date you will be required to upload a current proof of address document at the bottom of this online form.
To help us verify your identity as a CSB client please enter below the details of the photo ID you supplied when opening your CSB account, if your photo ID has changed since opening your CSB account please enter below the details for your current photo ID. Please note you will need to verify your photo ID with us by using our ID-Scan app if, you have never used our ID Scan app or it has been over 3 months since last using it, and/or you have changed your photo ID since opening your CSB account. Details on our ID-Scan can be found at the bottom of this online form.

Please download our ID-Scan mobile app to verify your proof of identification listed above. Once you have downloaded the app take a selfie of yourself and a photo of your identification listed above. At the end of the process make sure you check the results are successful (Facial liveness ticked and a facial score above 90) and submit it to Capital Security Bank.
